We are seeking dynamic forward thinking individuals for the following positions:

Practice Manager – One Position

Responsible for the day to day management of a team and an integral part of the reform of crisis accommodation services to young people.

You will be responsible to the Clinical Services Manager for the co-ordination, supervision and professional support of staff; and ensuring that practice standards meet all clinical governance principles in delivering crisis accommodation and related support services for young men, young women and children.

Case Manager - 3 Positions

The Case Managers are the direct link between the Practice Manager and Case Workers. You will be responsible for services to young Guests, service intervention and standards.

You will be responsible to the Practice Managerfor responding to each young person’s needs by being primarily responsible for case planning and case management; and providing leadership and direction to the team of Case Workers. Some evening shifts will apply.

Case Workers - 10 FTE Positions

The Case Workers are responsible for the direct support and service delivery, including keyworker responsibilities when allocated.

You will be responsible to the Practice Manager for providing an environment of care to enhance the social and living skills of young people; and meet the physical, emotional and developmental needs of young people in crisis accommodation. Shift work applies to all Case Worker positions

You will contribute to the delivery of services to young people that facilitates their development and progression towards:

    • Meeting young people’s Stay Strong Plans;
    • Age appropriate independent living;
    • Crisis interventions;
    • Supporting developmental aspirations, and
    • Personal growth, community involvement, and social and recreational activities.
